Eco SCUBA Key West's Express Check-In Service

This is EcoSCUBA's Express Check-in form. Here you can enter all of the information about yourself and your certification (Certifying agency, rating, eperience, etc.). This will save some time when you arrive for your scheduled dive trip with EcoSCUBA. It will also let us know your diving status for waiver purposes before you even arrive. When you come to see us, we will already have your documentation prepared for you before hand. This is just one way that we hope to better serve divers and make the entire process pleasant.

Please make sure that you read the following information carefully. These instructions are provided so that your Express Check-In experience goes smoothly. Thank you!

Follow These Instructions Carefully:

Complete the form with your personal information.
Save the form where you can find it on your computer and then return to our website.
Email us at info@ecoscuba.com with the completed form as an attachment.
That's it, you're all done!

You will have to sign your form when you arrive at the shop. Until then, you are all ready to go diving!
